This guide walks you through every section of the Personal Tax Questionnaire in Heard. The 1040 is the individual tax return. Completing this questionnaire accurately gives your tax preparer everything they need to file your Sole Proprietor business and personal return correctly and on time.

What you’ll complete in each section
The questionnaire contains 6 sections: quarterly tax payments, personal details, changes in 2025, deductions and credits, non-practice income and liabilities, and document uploads. Once complete, you'll share everything directly with your tax preparer.

Quarterly tax payments
Please enter any quarterly estimated tax payments you made and upload the corresponding confirmations or receipts. Even if you did not make a payment for a specific quarter, it is important to indicate that as well.
This step helps calculate what you still owe (or whether you’ve overpaid) and reduces the risk of underpayment penalties.

Confirm Details
Here you'll review and update your personal information, business details, and location, including the states where you lived and earned income, as well as where your clients were located.
You'll also confirm your filing status and flag any relevant business updates from 2025, like whether you filed 1099s for contractors or ran another business outside your therapy practice.
Healthcare coverage is part of this section, too. You'll note what kind of coverage you had and whether you had an MSA, HSA, or FSA. Upload any relevant documents here, including 1095s, 1099-SAs, or Health Insurance Marketplace information.
If you're filing jointly, you'll enter your spouse's details. You'll also note any dependents you're claiming for 2025.

Changes in 2025
This section focuses on significant personal or financial changes from the past year. A common one: whether you bought or sold a home in 2025. Your answers here help shape the rest of the questionnaire.
Deductions and credits
Here, you’ll answer questions about personal expenses that may reduce your tax liability. Common areas include home ownership, a home office deduction (where Heard will walk you through two calculation methods so you can see which works in your favor), business mileage, charitable contributions, dependent care expenses, college expenses, and farm ownership if applicable.

Your tax return reflects your full financial picture, not just income from your practice.
In this section, you’ll report:
- W-2 income (from another job)
- Own rental properties
- Own investments (stocks, bonds, investment properties)
- Contributed to or changed retirement plan
- Retirement contributions
Upload documents
Once you've completed the earlier sections, you'll see a personalized document checklist based on your answers. Common documents include W-2s, 1099s, brokerage statements, retirement account statements, mortgage interest statements, and proof of quarterly tax payments.
You can click the View instructions link to help you find what you need. If something isn't available yet, upload what you have and mark the rest as "Unsure, or don't have" or "Need follow-up." You can add additional documents later to Your Practice>Documents>Taxes, and your tax preparer can help clarify what's required before you file.
Where to enter phone and internet expenses
If you’re a Sole Proprietor, use Heard's Home Office Deduction Tracker for Sole Proprietors. This ensures they’re handled correctly based on your entity type.
Share with tax preparer
When you're ready, you'll send your answers and documents to your tax preparer. After reviewing your information, they'll prepare a draft for you to review. You can sign off on it or ask questions through the chat portal.

Do I need to finish the questionnaire in one sitting?
No. Your progress is saved automatically. You can leave and return anytime.
How long does it take to complete?
Most therapists and wellness providers spend about one to two hours in total.
What happens if I don’t finish before the deadline?
